WYRA Crew Registration Forms

Spring 2010 – Available now are interactive forms that you can complete online and they’re even easier this year! We’ve added an automatic email button on the last page of the forms so that it automatically emails the form to the appropriate WYRA personnel.

• Varsity Paperwork and Dues Deadline – February 9, 2010
• Novice Paperwork and Dues Deadline – March 9, 2010 (Novice Registration form needed by first day of novice practice 2/23/2010)

Just fill out all your “WYRA i-Forms” found below using Adobe Acrobat Reader (it’s free) and click the “Email Form” on the last page of the online form (excluding the Medical packet which will need to be completed by a physician). It can’t get any easier than that! Please note: bolded links are forms that we need!

Note: Returning rowers need only complete information that has changed since last season plus the USRowing Waiver found in the Registration Packet. We’ve included “Quick and Easy” Steps to help you complete the Registration process. “Quick and Easy” Steps to Completing Your WYRA i-Forms:

1. Complete i-forms (appear in bolded text below).
   1. New rowers  should complete the Novice Registration form immediately (a novice rower is a first-year rower). This form needs to be completed by the first day of Novice practice. Remaining forms (Dues Statement, Medical Packet and Registration  Packet) are due with payment by March 9, 2010.
   2. Returning novices and varsity rowers should complete the Dues Statement, Medical Packet and Registration  Packet. However, if you are returning from last season, you’ll only need to provide information that has since changed (e.g., phone numbers, addresses, etc.) plus the USRowing Waiver found in the Registration Packet.
2. Email Forms.
   Each form now has an automatic “Email Form” button on the last page of the form. After you complete the form,  simply press the “Email Form” button to have your email software automatically prepare an email to the appropriate  WYRA personnel with the attached form. You may also want to carbon copy your own email address so that you have a copy of email that is sent. Please note: the medical packet does not have an email button since it must be completed by a physician before being sent to WYRA. If you are having problems with the automatic “Email Form” button, simply copy the form onto your computer and send it manually to the appropriate WYRA personnel. The Dues Statement goes to mpena@wyra.org and all other forms should be sent to Faith Pizor.
3. Pay your dues.
   Either 1) use the easy PayPal form online or 2) complete the bottom part of the Dues Statement with your payment information and mail the Dues Statement to WYRA. We chose to add PayPal as an option because of its superior built-in security features that restricts sharing critical credit card information with multiple organizations. You do not need a PayPal account to use this feature. Just simply provide your credit card info as you would to any other credit card processor using PayPal’s secure server. Or you can mail the Dues Statement in with your payment.

That’s all there is to it! If you happen to run into problems completing the forms, make sure you have the latest version of Adobe Reader.

  • Novice Welcome Form – please read for important information regarding novice rowers.
  • Novice Registration Form (i-form) – interactive form that you complete online and email to Executive Director Faith Pizor.  Note: if you are not using the PayPal option, please complete the credit card information (or enclose your check payable to WYRA). Do not send your credit card information via email as it is not secure.

    • Dues Statement (i-form) – complete form and click “Email Form” button to attach form to  email  mpena@wyra.org. You can now pay your WYRA dues online using PayPal.  Please note: you do not need a PayPal account to use this method.

    Or pay over two months! This will be handled as a two-month subscription and PayPal will automatically handle the 2nd month installment 30 days after your first installment.
    
    
    • Medical Packet (i-form) – you can complete the preliminary info on your rower, then print the form. When physician completes form,  mail or bring it to WYRA
    • Registration Packet (i-form) – complete form and press “Email Form” button at end of the form to send. This will send an email to fpizor@wyra.org with your completed form as an attachment.
